Linky Party! Behavior Plans and Behavior Problems

I have tried several different things to discipline students for negative behavior. I came up with this behavior plan with the help of my counselor and it has worked great for those students that have a hard time consistently following directions.  I usually have 1-2 students on a daily behavior plan per year. I use the regular color system where you change the clips for normal behavior problems. This is what I use for more serious problems that need more attention.

I separate the day by subject so the students have smaller goals to reach and attain. They get a happy face in each square if they did well and a sad face if they did not. This book goes everywhere with them (lunch, and specials etc. ) I total up the points and that determines their grade for the day. Each student has different goals to accomplish depending on what problems they are having in class. This goes home every had and comes back signed every day. The get a sticker for every E that they receive and after so many they can go to the treasure box. (I didn't make the form downloadable because it follows my schedule but feel free to copy it!)


 I staple them in a little booklet so they all stay together and that way I have plenty of documentation for SST's and parent meetings. They must have serious behavior problems before they go to a behavior book.

If the student needs even more incentives because of serious behavior I will add on computer time or lunch with a buddy etc.  I think the positive incentives should outweigh the negative for students that are having a hard time with behavior.   3. I linked to my clip chart post - at the end I have a daily documentation form that teachers can use to show the child's movement on the chart throughout the day, not just what they ended up on. Great documentation for parents, RtI, etc. Even if I have a kid who needs something extra, they still move up and down on the clip chart as a member of our class.
